Student Reporting Forms

Academic Integrity Violation Report Form
Anyone who suspects potential violations of academic integrity is encouraged to report such behavior to the instructor and/or to the Office of Academic Affairs.

Code of Conduct Violation Report Form
Students, faculty, and staff may report a potential or actual violation of the Student Code of Conduct to the Office of Student Life. 

Formal Grade Appeal Form
Students appealing a grade in a course may submit a formal grade appeal in writing to the Dean of the College after going through the informal process with the instructor.

Student Grievance Form
Students should use this form to submit a formal grievance regarding issues that have not been able to be resolved through the College’s Student Grievance Procedures.
