All members of the Maria College community are welcome and encouraged to report incidents that they experience or witness that are academic (such as cheating on an exam) or non-academic in nature (such as fighting or any other violations of the Student Code of Conduct), as well as behavior or conduct of others that raises concerns regarding an individual’s mental health, such as symptoms of depression, mental illness, eating disorders, suicide, drug addiction, or other issues.
Students are welcome to speak privately with a College official who can help address the incident or issue and provide support as appropriate, including the Dean of the College, Dean of Students, Director of Counseling, Pastoral Minister, Title IX Coordinator, or other trusted official.
Individuals can file one of the following forms online by visiting www.mariacollege.edu/about-maria/policies-procedures, or in various offices on campus, including the Office of the Dean of the College, the Office of the Dean of Students, Campus Security, or Department of Student Affairs. All reports will be directed to the appropriate College official and the reporting individual’s privacy will be maintained to the extent possible.
Maria College General Incident Report Form
All members of the Maria College community are welcome and encouraged to report incidents that they experience or witness. The purpose of this report form is to notify College officials of incidents that are non-academic and non-behavioral in nature.
Sexual Misconduct Form
Maria College is committed to promoting a learning and working environment where all members of the College community feel safe and respected. Acts of sexual misconduct are contrary to the College’s educational mission and values, are harmful to others, and will not be tolerated by Maria College.
Maria College employees or students should use this form to submit a formal report to the Title IX Coordinator regarding issues that have occurred in violation of the College’s Sexual Misconduct Policy. Prior to completing this form, please review the Maria College Sexual Misconduct Policy. Reports of sexual misconduct (such as assault, stalking, or harassment) should be made directly to the Title IX Coordinator.
Student Grievance Report Form
Maria College is committed to resolving student complaints regarding their academic and college experience in a fair and timely manner.
Maria students should use this form to submit a formal grievance regarding issues that have not been able to be resolved through the College’s Student Grievance Procedures. Prior to completing this form, the reporting individual should be familiar with Maria College’s Student Grievance Policy.
Maria College is a member of the National Council for State Authorization Reciprocity Agreements (NC-SARA) and follows the complaint resolution policies outlined on the NC-SARA policies and procedures site. For students enrolled in fully online academic programs, who have exhausted Maria’s student grievance process, the student may choose to file a complaint with the Office of College and University Evaluation, New York State Education Department as outlined here. Please note, this does not include complaints related to grades or student conduct violations, both of which are to be fully addressed via campus processes, not through the NC-SARA complaint resolution procedures.
